Appointments and Insurance Information

Appointments -

 

We try to see all patients on an appointment basis, so that we can reserve time for you. We make every effort to honor all time commitments and request and ask that you extend the same courtesy to us. If you cannot make a scheduled appointment, give us at least a 24 hour notice, so that someone else can use that time. On occasion, emergencies arise and we will make every effort to contact you should there be any change or delay in your schedule.

 

Insurance information -

 

When you arrive for your appointment, please provide us with a copy of your insurance card and a picture ID. If your plan requires a referral from your PCP, please bring it with you to your appointment or have your primary care physician's office fax it to us (713) 661-3394.

 

All copays and coinsurance fees are collected prior to your visit. If your deductible has not been met, you may be required to pay for your visit.

 

We know that questions may arise on insurance matters and our billing specialist will be happy to assist you with questions and/or concerns. We make ever effort to verify your benefits and notify you of your financial responsibility.
